WIN Wellness Services (Health Home) provides whole-person care by coordinating physical health, behavioral health, and community support services to help individuals achieve better overall wellness. Our program empowers patients and families to actively participate in their care through health education, chronic disease management, prevention services, and personalized care planning. We support successful transitions from hospitals and reduce avoidable visits to hospitals and emergency rooms, improve access to medical and behavioral healthcare, and connect individuals to valuable community resources. Through comprehensive care coordination and ongoing monitoring, we help improve health outcomes and strengthen self-management skills.

Taking care of your health can sometimes feel like a lot to handle, especially when you are managing both your physical well-being and your mental health or recovery goals. That is where the Maryland Medicaid Health Home program comes in.
Despite the name, a Health Home isn’t a physical building, a clinic, or a residential facility. It is a supportive team of caring professionals embedded right inside of WIN services. Our goal is simple: to stand by your side and connect all the pieces of your healthcare journey so you can focus on feeling your best.
How a Health Home Supports You
When you join a Health Home, you get a dedicated care team that looks at the whole picture of your life. We are here to help you navigate doctors, organize medications, and even find community resources.
We provide support through six core services tailored entirely to you:
- Personalized Care Planning: We sit down with you to listen to your story and create a personalized wellness plan based on your unique goals and health history.
- Stress-Free Coordination: We help organize your schedules, remind you of appointments, and make sure all of your different doctors are talking to each other.
- Healthy Living Coaching: We provide gentle guidance and easy-to-understand education on physical wellness, nutrition, and managing everyday health conditions.
- Safe Transitions: If you ever have to stay in the hospital, we are right there to help you transition back home safely, double-check your new medications, and keep you from having to go back.
- Family & Loved One Support: We offer guidance and advocacy for you and your family, helping everyone understand your care and make confident medical decisions together.
- Connections to Community Resources: Your health is about more than just medicine. We link you directly to essential local resources like stable housing, food programs, and reliable transportation.
Who Can Join?
The Health Home program is a special, extra benefit available at no cost to Maryland Medicaid members who need a little extra coordination in their care. You may qualify if you are:
- An Adult managing a serious mental health condition or a substance use recovery journey.
- A Child or Teen experiencing severe emotional or behavioral challenges.
- Currently Enrolled in WIN’s psychiatric rehabilitation program.
Peace of Mind, Fully Covered
If you qualify, these comprehensive care coordination services are fully covered by Maryland Medicaid. There are never any out-of-pocket costs, hidden fees, or copays for your Health Home team. We are simply an extra layer of support here to help you live a healthier, happier life.
